Under the army point-list, it states that the Exarch for the Dire Avengers may take The Diresword, which is a power weapon, and a shuriken pistol as one choice. Then, another choice for him is the Shimmer Shield with a power weapon. Considering the Diresword is a power weapon, wouldn't it only make sense to equip the unit with both items?

no a diresword is a powerweapon that inflicts instant death if the wounded model fails a leadership test for each wound inflicted, which is better than the average power weapon. the shimmer shield is a 5+ invulnerable for the whole unit. so its a choice between protection for the whole unit or extra killing power
and the shuriken pistol also adds the exarch 1 extra attack with his diresword :).
tbh I'd never take the diresword over the shimmershield (5++ is hughe combined with defense exarch power! your 10 man squad of dire avengers is going to win combat against plain terminators on many a occasion).

If the diresword were stronger I'd consider taking it as a character or monster hunter, but as it is, I agree it's too unlikely to wound in the first place to be worth it compared to a shimmershield.

The shimmershield only provides the 5++ in close combat. Just a clarification.

Yes, perhaps it would make sense, as the Diresword's insta-kill dealy is so unlikely that its additional killing power becomes negligible.

True, but at least he still has some ranged capability, and the SP gives him an additional attack with the diresword.

Just to clarify Freefall's comment here, it would make sense (well, except for the cost), but it can't be done - each of the three upgrade options replaces the exarch's shuriken catapult, so you can only take one.

Yeah, but I really hate to waste a BS 5 by equipping the exarch with melee only capability.
Now if the 5++ was for everything? I'd seriously reconsider it. :)
