Grenades and Melta bombs; expendable?

So are krak/frag/EMP grenades a one time use item, or can you use it on every assault? Would the codex specify? What about melta bombs?

As a sub-question, how hard up are people about WYSIWYG when it comes to melta bombs and grenades? There aren't even any melta bombs on the CSM sprues, so I would imagine it isn't a big deal.

They are not a one time use item. They are in effect each time the situation arises.

As for WYSIWYG, Grenades/bombs have never been requried, even when GW was hard core about models having the right items.
