does GW make a Combi-Flamer or do I have to make one?

I don't actually recall seeing a combiflamer on anything. It should be pretty easy to cobble one together though. You could shave the top off a boltgun, mount an IG flamer to the top, and maybe a flamer fuel cell to the side or something.

There is a chaos vehicle one, but I don't think it would work very well for infantry. A conversion is probably the best way to go.

Chaos termies have one in the kit, I think loyalist termies might too. You're better off converting, it's cheaper, and the guns won't look so oversized. Check this one (http://www.games-workshop.com/gws/content/article.jsp?catId=cat180005&categoryId=900001&section=&pIndex=1&aId=2300006&start=2), not technically a combi-flamer, but it's a bolter with a one-shot flame attachment, sooooo...

The easiest conversion is the storm bolter cut on side of the barrel off then get a flamer and cut at the bottom of the tubes and glue to the cutoff barrels.
Only two models I know of other then chaos terminators have them it's Chaplain cassius and the limited edition SOB canoness.

lots of ways to handle this one i guess. i mount a flamer-head under the barrel on an ordinary boltgun, add some of the detailing from a heavy flamer to the side of the gun, then connect the two with a bit of wire.

It's curious however that if look at the drawing of cassius on the C:SM it seems (at least to me) he has a combi-melta even if the rules clearely state combiflamer.
