Need help identifying a model

I've got a 120mm Forgeworld marine and can't remember his proper name. My google-fu has failed me and the current Forgeworld catalog (and page) lack the information I need. Does anyone remember who this guy is? :confused::confused:

Yes, he was. Also a member of the Mortifactors and the Deathwatch. This sculpt is clearly of an Ultramarine, so it can't even be a version of Artemis.
