I am sure there have been a lot of posts about this but can you mix the new 6th discipline psychic powers with the old codex powers or is all or nothing? Could you please cite a source with your responses? Thanks

You can either take codex powers, or swap the codex powers a model has for an equal number of rulebook powers. The exception is powers that a model has innately, like hammerhands, in the case of GK. These are in addition to codex or generated powers (Troke, Ward et al., 2012). For example a GK librarian has hammerhands, and then can choose to either take codex or rulebook powers, but cannot mix and match.

I don't believe anyone can mix and match--however, the specific rules for taking powers out of the five core disciplines are handled on a faction-by-faction basis, and you'll want to check your faction's FAQ document to see how they work, specifically.

There is not, currently, a core set of rules which applies to all factions and covers the entire process.

There is nothing in the eldar FAQ other than eldrad can take 4 powers out of his known five. Therefore he is mixing automatically sinc he can only swap 4/5 powers

You don't swap power for power though, you use the book ones instead of the codex powers and then it specifies that you get 4.

Can anyone point me to a excerpt from the rule book? I read it thoroughly and could not find anything either way

Pretty sure it's all covered in the FAQs, each one gives the options for the models that can swap out powers in that army.

It does not have this clause in the rulebook only the race specific FAQs. Here is an excerpt from Eldar FAQ:

"'Psychic Powers
A farseer (includind Elrad Uilhran) may use the psychic disciplines found in the Warhammer 40,000 rulebook, instead of those in Codex: Eldar. If he does so, for each psychic power he has purchased from Codex: Eldar, generate a new power from either the Divination or Telepathy discipline (in any combination) before armies are deployed. Elrad Ulthran generates four new powers."'

Each FAQ has a different clause for evey army, but all say it is an all or nothing swap.

That does indeed seem to be the case--it is an all or nothing swap for Eldar, certainly, including Eldrad. It was my impression that this is true for all codices, but I haven't actually read all of the FAQs.
