Where do modelers get the pre-heresy style Space Marine heads, like what were used in the Warhammer 30,000 BoLS codex? Are you carving them out of the "armour through the ages" marines?

I just had a discussion with Bigred about this, yesterday. I am doinng a pre-Heresy SW army. He showed me the kit they were using, it is the FW Red Scorpion Upgrade kit.

There are some gems spread across ranges. The best bulk buys are the Red Scorpion kit from Forge World and the head set from maxmini.eu (but they make even FW delivery look speedy!).
Across other GW sprues, there's 1 on the Black Templars, 1 in the Iron Warrior Metal upgrade kit, Khorne Berserker heads with the ears removed, Chaos Space Marine heads with some cutting work.
Heck, you could use Bretonnian knight helms like I did below:

i concur with re-modelling chaos marine heads, i mean, you can't get much more pre-heresy than the armour from the heresy itself...

I'll have to look into that bretonnian head conversion, as I have several spare helmets in my bitz box. Is the studded shoulder pad from the chaos sprue, or is it just gs dots on a normal shoulder pad?
Zou get the studded shoulder pads in the marine boxes or zou can convert the studs with small pin heads.
Drill some holes and cut of the pin heads so that the shorter pin fits in the drillholes.
SO zou can ad studs easilz all over the armour.

That particular pad, typical of me, was the whole am from the limited edition White Dwarf space marine you got for the subscription. I like butchering rare models :)
The whole model was:
Bretonnian helmet front.
Berserker legs.
Red Scorpion chest.
White Dwarf marine chain sword arm.
Regular plastic marine bolt pistol arm.
Studs were added using lots of layers of pva dots using a cocktail stick.
There's a horned chaos marine helmet that looks very nice without horns - for artificer armour or the likes of Salamanders/Death Guard. I used it on my PH-spin Vulkan:

Apoc masters of the[...] pack has 1 chap holding a very nice Mk3 helmet on his fist. It's a ballache to cut off though, but:

I have also used resin heads from MaxMini to good effect; they have a line which look pretty inspired by pre-Heresy artwork: http://www.maxmini.eu/store/index.php?main_page=product_info&cPath=12&products_id=60
If you email them nicely you can even pick which helmets you get, instead of a random assortment. :)

We used a lot of different heads. berzekers, chaos marine heads, regular marine heads, Black Templar "Crusader" pattern heads...the list goes on.
My personal Favorite heads were Warriors of Chaos - Chaos Warrior heads from the fantasy range. Just clip the horns and file them down. They have a great 'old school' look to them.
But what it really boils down to is what you think looks cool and it fun to convert. Thats why we here in austin played the armies in the first place :) Hope this helps!
